Join a respected and supportive healthcare organization as a Registered Nurse (RN) within a Family Medicine department. This is an excellent opportunity for a compassionate, patient-focused nurse to provide comprehensive outpatient care while working alongside experienced physicians, advanced practice providers, medical assistants, and administrative staff. The Family Medicine RN will play an important role in supporting patients of all ages, coordinating care, providing education, managing clinical communications, and helping ensure a welcoming, efficient, and high-quality patient experience. Full time: M-F 8:00am - 5:30pm

Location: Lake Grove, NY

Key Responsibilities

• Provide direct nursing care to patients in a family medicine and primary care setting.
• Complete patient assessments, obtain and document vital signs, review medications, allergies, medical history, and reason for visit.
• Triage patient phone calls, portal messages, and clinical concerns; assess urgency and escalate concerns to the appropriate provider when needed.
• Administer medications, injections, vaccines, and treatments as ordered and within scope of practice.
• Assist physicians and advanced practice providers with examinations, procedures, and patient-care coordination.
• Educate patients and families regarding diagnoses, medications, treatment plans, preventive care, chronic disease management, diet, lifestyle changes, and follow-up instructions.
• Coordinate referrals, diagnostic testing, laboratory work, imaging, and follow-up appointments.
• Review and communicate provider-approved test results and care instructions to patients.
• Maintain accurate, complete, and timely documentation in the electronic medical record.
• Monitor and follow up on outstanding orders, referrals, laboratory results, medication refill requests, and patient concerns.
• Collaborate with interdisciplinary team members to support continuity of care and positive patient outcomes.
• Maintain exam-room readiness, ensure appropriate supplies are available, and follow infection-prevention standards.
• Adhere to HIPAA, OSHA, organizational policies, and applicable New York State and federal regulations.
• Participate in team meetings, quality-improvement activities, staff education, and other departmental initiatives.

Qualifications

• Current, active New York State Registered Professional Nurse license and registration required.
• Associate Degree in Nursing required; Bachelor of Science in Nursing preferred.
• Current BLS certification required.
• Previous experience in family medicine, primary care, internal medicine, ambulatory care, pediatrics, urgent care, or a related outpatient setting preferred.
• Strong clinical assessment, triage, patient-education, and care-coordination skills.
• Experience administering vaccines, injections, medications, and treatments in an outpatient setting preferred.
• Proficiency with electronic medical record systems and standard computer applications.
• Excellent verbal and written communication skills.
• Strong organizational skills, attention to detail, and ability to manage multiple patient-care priorities.
• Ability to work independently while also contributing effectively within a collaborative clinical team.
